The medical center had six doctors and one clinic. Appointments were by phone only. The line was busy from eight in the morning, so someone in real pain waited behind a routine visit. The receptionist wrote capacity in a diary. If two people called at once, one hour was filled twice and one of the two patients arrived to find they had no appointment.
No-shows were high. The reminder depended on that same receptionist, and on a busy evening nobody sent a message. The doctor saw an empty hour, and the patient said the next week that nobody had reminded them. Moving a visit from one doctor to another meant crossing out the diary and hoping the second receptionist had room.
Each doctor has a capacity for the day, and the site shows only open hours. Two people cannot take one slot. Booking, cancellation, and a move sit on the same calendar. The reminder goes out automatically on the morning of the visit. It does not depend on a busy evening.
If the patient does not come, the hour opens for the wait-list. The receptionist moves the visit in the system, and the destination doctor's capacity is known at once. The clinic manager reads each doctor's no-show rate from a report, not from a guess in the corridor.
The night before, the patient picks the internist and sees only tomorrow's open times. The booking is stored and a confirmation text goes out. A separate reminder goes out in the morning. If the patient cancels ten minutes before, the first person on the wait-list takes that hour.
The receptionist moves a patient who insists on changing doctor onto the second doctor's calendar. If that hour is full, the system shows the next one, and nobody's diary ends up with two names on one line. At noon the manager sees that the dermatologist's no-show rate is higher, and an extra shift is decided from that report.
The phone line is freer for urgent cases, because routine visits are booked on the site. An hour is not sold twice, and missed visits dropped because the reminder is systematic.
The clinic still gets busy. The busy-ness is in the report the same day, not after the queue has passed the door. Capacity is what the calendar shows, not what the receptionist remembers.
